Gaming Employees Forms
A gaming employee application is to be completed by those wishing to perform gaming duties in a club or hotel. This licence is issued for 5 years. Renewal notices are sent to the last known postal address 2 months prior to the expiry date of the licence. Renewal applications are not accepted after the expiry date of the licence.
Key Monitoring Employees Forms
A key monitoring employee application is to be completed by staff employed by a Licensed Monitoring Operator. This licence is issued for 5 years.
Renewal notices are sent to the last known postal address 2 months prior to the expiry date of the licence. Renewal applications are not accepted after the expiry date of the licence.

Gaming Nominee Forms
A gaming nominee application is to be completed by those nominated to be a gaming nominee at a club or hotel. They are responsible for the conduct of gaming at the venue. This licence is issued for 5 years.
If the nominated person currently holds a gaming employee licence, the Form 65 - Fast Track Upgrade form can be used.
It is a requirement that all applicants for a gaming nominee licence attend an approved gaming nominee training course conducted by Clubs Qld or QHA.
Renewal notices are sent to the last known postal address 2 months prior to the expiry date of the licence. Renewal applications are not accepted after the expiry date of the licence

Repairers Forms
A repairer application is to be completed by those who are installing and servicing gaming machines for a licensed service contractor. This licence is issued for 5 years.
